RHB Revises X-Ray Machine Registration Form in California – Dental Compliance
The California Department of Public Health Radiologic Health Branch (RHB) recently revised the radiation machine registration form, RH2261, which must be completed and submitted to RHB. Three forms are now available for registration: 1) RH2261N for new registrations, 2) RH2261C for a change to a registration, and 3) RH2261W for registrants who no longer have an x-ray machine. Registrants who have pre-printed copies of the old RHB2261 form should discontinue using them. Dental x-ray machines must be registered with RHB within 30 days of acquisition, change of name, address, location, sale or purchase of equipment.
